COVID-19 Updates

Dearest patrons,

During this time, we continue to operate with the well-being of our employees and customers as our top priority. Here are some of the supplemental measures we’ve introduced to keep everyone safe:

• All Forma employees will be using protective gloves and face masks.

• Employees will also have their temperature checked upon entering the workplace.

• Dine In (@25% capacity), Pickup, takeout, and delivery service is available. Please place your order over the phone or through one of our delivery partners (Caviar, DoorDash, Seamless/Grubhub). When picking up food, look for your order on one of the three labeled tables located at the front of the restaurant. Your food will be in a sealed bag with your name or order number attached to it.

• Indoor/Outdoor dining is still available on a first come, first served basis
